The phone icon that comes up on the appointment screen represents a clients appointment that has had a reminder automatically sent to them.

This will appear automatically when the reminder has been sent to the client.

Appointment reminders can be set by clicking on Marketing and then Appointment Reminders

Set the SMS reminders in the SMS tab

and Email in the Email tab:
