Video: Reviewing a Consultation Form in the Client Card

Step by step guide

Click Clients on the left hand side of the screen.

Search the Client Details in the search bar. Choose the client by double clicking on the name.

In the client card, go to Consultations > Forms

To view a form on desktop, click View Form

The form will appear. It can be scrolled downwards to view all questions and details. At the bottom of form you will also see the client's digital signature.

Reviewing a Completed Consultation Form on Phorest Go

You can view a clients completed Consultation Form from within their client card on Phorest Go.

Swipe from the left of the screen and tap Clients.

Search for the client to bring up their client card then tap Forms.

From there, you can view any completed Consultation Forms.
